Social Worker

Te āheinga - The opportunity:

Reporting to the Clinical Lead for Te Pou Oranga Whaiora at Te Whare Ora o Kōpūārau, the Kaiārahi Navigator acts as an “on the ground” advocate for whānau to access integrated care and support services.

You will have a Social Work or clinical qualification or will have commenced your study pathway.

Ngā Kawenga me ngā Mahi – Duties & responsibilities:

  • Work with whānau in your allocated case load to understand their needs, aspirations, support, own potential, and barriers and introduce wrap around services and support.

  • Complete pre-entry, referral, intake, triage, assessments, and discharge processes in line with the required framework.

  • Collaborate and work with internal and external providers, Kaimahi and relevant community services to complete the service delivery requirements.

  • Report on, evaluate, review, all goals, plans and milestones to ensure the service delivery is to the required standard and fit for purpose to achieve whānau goals.

Ngā wheako me ngā pūkenga - Experience & Skills required:

  • A qualification in Social Work or in the final year of study towards a recognised Social Work Degree.

  • An understanding of the social work framework and how to apply this as a Kaiārahi Navigator.

  • Act with a high-level of professionalism, able to prioritise, work under pressure and maintain accuracy.

  • Passionate about empowering our whānau towards Te Ao Marama.

  • Knowledge of Te Tiriti o Waitangi.

  • Knowledge of Te Whakatōhea me ona tikanga would be highly regarded.
